WHO YOU ARE

The Performance Consultant (PC) role works directly with network partners to ensure successful implementation and execution of Healthy Alliance contracts. PCs are responsible for managing partners’ participation and performance, assessing needs, and providing strategic development of plans to align partner performance with successful implementation of contract deliverables.

WHAT YOU’LL DO

  • Oversee partner contract and performance management.
  • Conduct partner risk assessments and identify any barriers that impact meeting contract deliverables and resolve issues, when appropriate. 
  • Identify partner needs and connect to appropriate Healthy Alliance stakeholders. 
  • Track and communicate monthly partner performance to internal and external stakeholders.
  • Maintain current knowledge and understanding of Medicaid and local transformation, including Waiver programs, Triple Aim, and value-based purchasing.

WHAT YOU’LL NEED

Education 

  • Bachelor’s degree required. Significant and relevant work experience may be accepted in lieu of educational experience. Degree in health, social services, or a related field preferred.

Professional Work Experience

  • Consulting, account management, health or business administration, social work, human service, or nursing experience preferred.
  • Experience with clinical process improvement and performance improvement.
  • Advanced knowledge and understanding of social drivers of health, community-based organizations (CBOs), clinical care settings, Delivery System Reform Incentive Payment (DSRIP) program, and value-based purchasing (VBP).
  • Experience with health systems and MCO organizations. 

Skills, Knowledge, and Abilities 

  • Ability to lead others in a team-based environment with dedication to support co-workers.
  • Comfortable working in a fast-paced, dynamic environment while maintaining focus on key goals.
  • Excellent organizational skills.
  • Excellent oral and written communication skills. 
  • Ability to work independently to manage multiple projects and activities with little to no supervision.
  • Familiarity with process mapping and workflow analysis tools.
  • Proficiency in Microsoft Office applications including Excel, Access, and PowerPoint. 
  • LEAN experience/certification preferred.

    WHO YOU ARE

    As a Referral Navigator, you’ll be responsible for receiving, prioritizing, monitoring, and triaging referrals that require service connections across community-based and clinical organizations. This position is also responsible for working within the Healthy Alliance Solution Suite, including Unite Us and the Ride Health transportation platforms. This position is directed by the Referral Coordination Manager.

    WHAT YOU’LL DO

    • Manage incoming referrals received by the Referral Coordination Center, using various technology platforms to ensure successful and timely connections are made between clients (community members) and the appropriate community partner that best meets community members’ needs.
    • Maintain effective communication with community members and/or referring organizations to ensure acceptance and resolution of referrals by the receiving organization or to redirect the referral to another provider, if necessary.
    • Document progress and action taken with each referral, as detailed in the Network Standards.
    • Report-out on partner effectiveness and efficiencies to the Referral Coordination Manager or the appropriate contact and escalate unresolved community member/service issues timely.
    • Share concise feedback on the Solution Suite’s successes and challenges and continue to look for opportunities to enhance and simplify the user experience.
    • Manage content and maintain processes to support effective partner communication with updated marketing materials/collateral.
    • Actively engage in learning and sharing knowledge to support new products that will be rolled out as part of the Healthy Alliance Solution Suite.
    • Some local travel (20%) to local assigned partner sites and/or community events.

    WHAT YOU’LL NEED

    Education 

    • Associate degree in health, social services, or related field preferred. Equivalent work experience in a related field may be considered in lieu of degree requirements.

    Professional Work Experience

    • Minimum of 5+ years related experience in a clinical, non-profit, or managed-care organization environment preferred.
    • Extensive knowledge and understanding of health equity, social drivers of health, and social care data.

      WHO YOU ARE

      The Technical Business Analyst evaluates business processes, functions, and procedures to determine needs and develop strategies to drive business decisions that are likely to lead to the best return on investment.  

      WHAT YOU’LL DO

      • Elicit business needs and reporting requirements using multiple methods (e.g., interviews, document analysis, surveys, site visits, business process descriptions, use cases, scenarios, business analysis, competitive product analysis task, workflow analysis, and/or requirements workshops). 
      • Analyze business requirements and translate into system requirements. Create documentation regarding system specifications, RACI matrices, use cases, sequence and workflow diagrams, future state proposals, testing plans, issue logs, project plans, and more.  
      • Collaborate closely with developers on implementation, configuration of solutions, and prioritization of business value. 
      • Develop and execute test plans to facilitate quality control processes.  
      • Manage requirements traceability and changes to requirements through effective use of change control processes and tools. 
      • Conduct analysis and design at a complex level with consideration for people, processes, and systems while focused on efficiency and quality improvement.
      • Support data management to ensure data is complete, reliable, and valid. 
      • Identify opportunities to streamline and automate processes, develop process improvements, or re-engineer as needed to meet the long-term business strategies. 
      • Provide advice and guidance on best practices in support of new business and system implementations, including integration with existing processes and systems while maintaining adherence to contract terms. 
      • Build and maintain collaborative relationships with internal and external customers and stakeholders. 
      • Facilitate meetings and presentations to diverse audiences (i.e., stakeholders, customers, etc.). 

              WHAT YOU’LL NEED

              Education 

              • Bachelor’s degree required in information systems, systems analysis, computer science, engineering, science, finance, economics, statistics, or related fields.  

              Professional Work Experience

              • Minimum of 3-5 years of experience in business analysis, data analysis and reporting, and software testing required. 
              • Experience in health care or social care services industries is preferred. 

                Skills, Knowledge, and Abilities 

                • Ability to implement vendor software and related technical experience required. 
                • Experience with software development lifecycle and agile development required. 
                • Experience and proficiency in Microsoft Office (i.e., Word, Excel, PowerPoint, etc.). 
                • Experience and proficiency in flowchart and diagram tools (i.e., Microsoft Visio, Lucidchart, etc.). 
                • Report development experience using database and reporting tools, including SQL, Tableau, and Power BI preferred. 
                • Demonstrated experience using current business process management and automation tools. 
                • Familiarity with AWS and Azure. 
                • Excellent written and verbal communication skills with the ability to articulate business and technical concepts among both internal and external customers. 
                • Ability to understand and negotiate needs and expectations of multiple stakeholders through formal and informal methods. 
                • In-depth understanding of organizational data flow and its use in management decision-making required.  
                • Demonstrated analytical, critical thinking, and problem-solving abilities.
