Overview

Schenectady County Chapter of NYSARC, Inc., The Arc of Rensselaer County, and The City Mission of Schenectady aim to reduce avoidable hospital use among the at-risk populations of Medicaid recipients whom they serve. They plan to do so by expanding on a program which is already employed by the Arc of Rensselaer County, providing telehealth services to patients for access to round-the-clock emergency medical physicians who can order labs, make diagnoses, prescribe medications, and issue documentation in real-time. The project has a proven viability given that impoverished or disabled individuals face exceptional difficulty in accessing timely care, especially on nights or weekends. The Arc of Rensselaer County’s existent program has already successfully reduced emergency room and urgent care visits among their patient population by 20%, saving valuable resources while still adequately addressing patient health concerns.
