Alliance has partnered with Unite Us, a Veteran-owned technology company as the software provider that will power Alliance’s coordinated community-wide care network. We’ve named our network “Healthy Together” to call to our desire to have this tool serve our entire community and to live beyond the DSRIP program.
The Unite Us platform will allow Alliance’s clinical partners throughout Albany, Fulton, Montgomery, Rensselaer, Saratoga, and Schenectady counties to connect in real time with social service providers such as housing and workforce development agencies, behavioral health providers, and food pantries.
All partners across the network will be centrally connected to each other, with the ability to:
- Electronically connect patients to both clinical and social service providers.
- Track patient progress & receive automated feedback from partners to ensure care and services are received.
- Collaborate with community-wide “care teams” ensuring providers are on the same page regarding their shared patients’ progress.
- Collect data including structured patient outcomes to measure the network’s impact, including time to service and effectiveness of care.
Timeline and Initial Participants
Alliance, through its partner-driven Integrated Delivery Performance Workgroup, will begin implementation planning and initial beta group recruitment in March 2018. An initial group of 50 service organizations covering a diverse range of services will be selected in order to ensure a robust and effective launch.
