At Healthy Alliance, we lead the way in transforming how health care is delivered, shifting focus from singular medical interventions to a more proactive approach that addresses the whole needs of a person on a much larger scale. By addressing health from all angles, we are cultivating the type of change that improves health and empowers the underserved.

COMMUNITY RESOURCES
TO MEET YOUR NEEDS
People have complex health-related social needs – whether it’s limited access to healthy foods, a safe place to sleep, or warm clothing – requiring streamlined coordination of care between a variety of providers. Our local team works to get you connected to resources that are nearby (healthy food, help with doctor’s appointments, finding a counselor, getting enrolled in an educational program, and more) for free.
With ~1,300 partner locations spanning 25 counties in New York State, we believe everyone should have the same opportunity to be healthy.

#DidYouKnow?
When you log into Unite Us in the North Country, Central NY, and the Capital Region, we’re the ones working behind the scenes supporting successful connections to care.
What does that mean? To keep it simple, we’re among one of the only one’s doing it all — managing the social care network, overseeing the technical infrastructure, managing screenings and referrals from start to connection, aggregating and analyzing data, filling network gaps based on community needs, offering support services to partners as extensions of their teams, and much more — across 30 counties.
Thanks to the 4,700+ active users and 2,350+ programs on the platform, health care delivery is continuously being transformed – one referral at a time.
